The only plugin that I know of that allows you to assign access teams. The CSV should only contain the list of users (mail or domainname) https://www.youtube.com/watch?v=KXDdPZRi1DU

This has been reported over a year ago that the template is no longer available for download and was never added to GitHub either. This app is unusable without the template and should be removed.

Nice tool to add the users into the team or change the business unit of multiple users. Remove the existing teams option is not working and the downloaded template is Excel file even though .csv file is supposed to be uploaded.
